1. If you are having problems connecting to Athena and Accelerated Reader (if applicable), please consult and follow the Network Troubleshooting page in your Technology Notebook. If you cannot find it, contact LPC and they will fax it to you. If this does not solve the problem, continue with this form.

2.  From the options below, please select the problem(s) that best fits the situation on your computer.

If you would like to briefly describe your problem, select a symptom, and then enter your description in the space provided below. Please be specific in your description. Do not write that your computer does something when you push enter. Provide an example of what you were doing at the time of the problem.

Logon error.
Athena error. (Please note error code below.)
Other Athena error (Please describe problem in space below.)
Athena is inaccessible from more than two computers.
Unable to connect to Internet.
No picture on monitor.
Computer freezes. (Either at startup, logon, WinU opening, etc.)
WinU error. (Please describe what is happening and when below.)
Virus message. (Please note full virus name below.)
Battery backup replacement. (Replace light is red.)
Error message on serve.r (Call LPC immediately.)
New Dell setup.
Other. (See below.)

3. If your problem is not listed in the above choices, please select Other and type your problem in the provided area below. Please be specific and provide details.

 

4. Click Submit to send this request to Library Processing or Clear to restart the form. Once you have submitted the request, print the confirmation page that follows. Tape this to the monitor of the affected stations for easy identification.
